You can always use the default WordPress contact form – however if you want to add fields there are other options.
I have used cformsII which is a powerful and feature rich form plugin for Wordpress, offering convenient deployment of multiple Ajax driven contact forms throughout your blog or even on the same page. I have tested other but they were not for free or didn’t offer the options and features I needed. Therefore, it is the only contact form I have graded.
It is not the easiest to get a grip of, but with that many features included it is worth the efforts
